Edgewood Center for Children and Families is Hiring a Remote Family Partner

NOTE: The California Department of Public Health (CDPH) has issued new requirements for health care and congregant care facilities to decrease the risk of COVID-19 outbreaks. Based on the CDPH public health mandate, all candidates for positions at Edgewood Center must provide proof of vaccination. Edgewood Center will consider applicants with ADA and Title VII exemptions on a case-by-case basis.

Family Partner-TAY- This is a full-time, non-exempt, hybrid position, M-F, 9am to 5pm (with flexibility).
Program Description
The Transition Age Youth (TAY) Full-Service Partnership (FSP) Program provides intensive community-based services to 45 young adults (ages 16-25) identified as having the highest mental health-related needs in San Mateo County and relies on a diverse staff and innovative program model to effectively meet the needs of this underserved and often marginalized population. Specific supports and services provided by this team include case management, mental health treatment (assessment, therapy, medication management, and psychiatry), family support, crisis prevention and intervention, skill building (independent living, vocational, community safety, and emotional/behavioral), socialization and recreational activities, peer and family relationship building, academic support and coordination, employment exploration, and housing support.
Position Description
The Family Partner is a formal member of our multidisciplinary team whose role is to serve parents, caregivers, and non-professional supportive adults of our 16-25-year-old. Family Partners use their own personal experience as a family member of a child or young adult who has special needs and has understanding of the many child and family serving systems, these may include schools, mental health, physical health/medical, juvenile justice/legal, and/or child welfare systems. The Family Partner also builds relationships with community groups and providers and supports caregivers/family members in accessing the services, provides coaching, advocacy, and linkage to community and family supports. In addition, Family Partners coordinate/facilitate support and skills groups for caregivers, family members, and any other individuals our young adults consider “family.” The Family Partner works closely with other Family Support and Adjunctive team members to provide high quality care to the chosen family members of our transition age youth.
Position Core Functions
  • Ability to partner with team members, young adults, and family members and/or supporters in the creation of holistic, individualized care plans.
  • Drawing from lived experience, provides training, consultation and peer support to family members, caregivers, and non-professional supporters of young adults in the program.
  • Encourage and support family members, caregivers and supporters in sharing their voice and perspective at family conferences, community meetings, trainings, and conferences.
  • Utilizing feedback from young adults and their families, co-leads the creation and implementation of activities and events focused on building and enhancing their relationships.
  • Creates and facilitates small group workshops, discussions, and learning opportunities for family members and caregivers.
  • Ability to develop, maintain, and strengthen professional relationships with supportive family networks, services, and resources.
  • Calm and professional response to crisis situations, including the use of mediation, de-escalation, and self-care techniques.
  • Cultivates and maintains a structured independent work schedule that allows accessibility to clients, team members and supervisor while in the field.
  • Provide mentoring and coaching in advocacy and empowerment skills.
  • Group and meeting coordination and facilitation skills.
  • Provides mentoring and coaching of resource acquisition, self-advocacy, self-care, and parenting skills to assigned family members.
Essential Functions
  • Completes role-specific documentation and paperwork thoroughly and in a timely manner, meeting expected timeframe and productivity requirements.
  • Regards 16-25-year-old as persons with dignity and competence and engages them as full collaborators in service planning, delivery, and evaluation.
  • Works one on one with family members and/or caregivers at Edgewood locations, in the home, and/or their community settings, depending on the needs and requests and the community support structures.
  • Attends and actively participates in all mandatory meetings, including staff meetings, client team meetings, and treatment planning meeting.
  • Understands and values cultural and racial differences, their alternative perspectives on mental illness, help-seeking and alternative healing practices, as well as lifestyles, goals, family, and community life.
  • Consistently and effectively communicates with all team members (young adults, families, colleagues, natural supports, and providers), on an on-going basis, ensuring others have necessary and up-to-date information about client engagement, symptoms and behaviors, and progress towards goals.
  • Completes individualized plans for each assigned client, ensuring goals, interventions, activities, and progress are all monitored and documented.
  • Cultivates and maintains a structured independent work schedule that allows accessibility to clients, team members and supervisor while in the field.
  • Completes program and agency-specific documentation to meet San Mateo County ,Medi-Cal and The Joint Commission standards.
  • Facilitates a safe and welcoming environment in the workplace, community, and vehicles (agency or personal).
  • Maintains confidential health records of young adults in compliance with the Edgewood, EPSDT, and San Mateo County Behavioral Health standards and regulations.
  • Attends and actively participates in 1:1 supervision; enlists support, requests and integrates feedback.
  • Maintains positive work relationships in a respectful and collaborative manner.
  • Collaborate and communicate with internal and external stakeholders, in a manner that promotes agency mission, vision, and values; agency, region, and area goals; consumer and family satisfaction; and highest quality care.
  • Other duties as assigned.
Hourly Rate Range Information and Benefits:
This position has an hourly rate of $24.75-$27.23 depending on bilingual language skills.
